I, the cardholder authorized to make charges on the above credit card (the “Card”), hereby authorize Ojakian Tennis, Inc. to charge my Card for tennis instruction and related services provided to me, in accordance with Ojakian’s standard billing policies and procedures. Ojakian will bill me on the 15th and last day of each calendar month. I agree that if my Card becomes invalid for any reason, I will provide Ojakian with a new, valid credit card for billing purposes. Ojakian Tennis will attempt to contact me if an appointment must be cancelled due to rain or instructor unavailability, and I will not be charged for any appointment that Ojakian cancels due to instructor unavailability or weather. Notwithstanding, I agree to be charged for any appointment I cancel on less than twenty-four (24) hours’ notice.
